Wisconsin releases enforcement actions for April

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for April.

The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Parmeet Arora, 8176 Basil Ct., Neenah, WI 54956, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

W C Askew, 11230 Park Meadows Dr., Apt. A, South Haven, MI 49090, had his application for an insurance license denied for 90 days. This action was taken based on allegations of failing to disclose administrative actions taken by other states.

Dawn Bieda, 500 9th Ave., Union Grove, WI 53182, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Matthew Cyr, 941 Rumley Run, Deforest, WI 53532, had his license suspended for one year and was ordered to pay a forfeiture of $10,000. These actions were taken based on allegations of making misrepresentations related to an insurance contract.

Jason Donnelly, 2024 Fuschia Ct., Tega Cay, SC 29708, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report administrative actions taken by other states.

Wade A. Elder, 2812 Main Ave., Sheboygan, WI 53083-3752, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report a criminal charge/conviction to OCI.

Natalie Finkley, 4067 S. 70th St., Milwaukee, WI 53220, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Todd Gilley, 13340 Southview Ln., Dallas, TX 75240, had his application for a license denied. This action was taken based on allegations of having a criminal conviction which may be substantially related to the circumstances of holding an insurance license.

David Gilliam, 3934 N. 65th St., Milwaukee, WI 53216, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

James Hardill, 7501 W. Oakland Park Blvd., Ste. 306, Tamarac, FL 33319-4974, stipulated to the permanent revocation of his non-resident insurance intermediary license. This action was taken based on allegations of misrepresentation and unsuitability.

Kimberly R. Harp, 301 E. Lamar St., Unit 10, McKinney, TX 75069, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state.

Francisco J. Bautista, 146 N. Elm St., Kimberly, WI 54136, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose a criminal conviction, failing to disclose an administrative action, and having a criminal conviction which may be substantially related to the business of insurance.

Natalie Lers, N8447 Swamp Rd., Manawa, WI 54949, agreed to the issuance of a one-year probationary insurance license. This action was taken based on allegations of failing to disclose a judgment for money owed to an insurer and failing to report a pretrial hearing when previously licensed.

Eric D. Malone, 5706 Logancraft Dr., Dallas, TX 75227, had his insurance license revoked. This action was taken based on allegations of failing to report a criminal conviction, having a criminal conviction that may be substantially related to the circumstances of holding an insurance license, failing to respond to inquiries from OCI, and failing to participate as ordered in an administrative hearing.

Christopher Meagher, W213N16804 Industrial Dr., Jackson, WI 53037, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Marshal Noeller, W3983 Krueger Rd., Appleton, WI 54913-9584, was ordered to pay a forfeiture of $1,000.00. This action was based on allegations of providing incorrect information on several applications for insurance.

John W. Offutt, 508 Solar Isle Dr., Fort Lauderdale, FL 33301-2626, agreed to pay a forfeiture of $1,000.00. This action was taken based on an allegation of misrepresentation in an insurance transaction.

David Phillips (Phillips Financial Services), 2200 E. Williams Field Rd., Ste. 200, Gilbert, AZ 85295, had his application for an insurance license denied. This action was taken based on allegations of providing misleading or incomplete information and failing to disclose administrative actions on the license application.

Rashadd Phillips, 1325 Annunciation St., New Orleans, LA 70130, had his insurance license revoked and was ordered to pay a forfeiture of $750.00. These actions were taken based on allegations of providing inaccurate information on a licensing application and failing to respond to inquiries from OCI.

Adrian Raventos, 4425 SW 160th Ave., Apt. 109, Miramar, FL 33027-5741, had his insurance license revoked. This action was taken based on allegations of failing to respond to a notice of hearing and failure to appear at a prehearing conference.

Trystin Reichhoff, 2844 Village Rd., Portage, WI 53901, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance marketing conduct.

Shanna M. Reid, N78W14573 Appleton Ave., Ste. 258, Menomonee Falls, WI 53051, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Erin M. Thornton, N7909 986th St., River Falls, WI 54022, stipulated to the permanent revocation of her resident insurance intermediary license. This action was taken based on allegations of misrepresentation and failure to respond to OCI requests for information.

Valencia Trice, 2818 Coff Ct., North Las Vegas, NV 89081-2443, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Patricia Wingard, 2937 Lighthouse Way, Conyers, GA 30013, agreed to a surrender of her license. This action was taken based on allegations of failing to disclose a criminal conviction and failing to respond to inquiries by OCI.

Kayla M. Woodcock, 25457 Elmer Ladner Rd., Pass Christian, MS 39571, was ordered to pay a forfeiture of $250.00. This action was taken based on allegations of failing to disclose a criminal conviction which may be substantially related to the business of insurance and failing to timely report an administrative action taken by another state.

Kevin Zinke, N9327 S. Lake Park Rd., #17, Appleton, WI 54915, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Allegations and Actions Against Companies

Interstate Medicare LLC, P.O. Box 51323, Albuquerque, NM 87181, was ordered to pay a forfeiture of $1,000. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

J Alejandro Fuentes Insurance Agency LLC, 1723 Spruce St., Grafton, WI 53024, agreed to surrender its license and pay a forfeiture of $250.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

The Lor Agency LLC, 12420 W. Hampton Ave., Unit 672, Butler, WI 53007, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

OTR Risk Management LLC, PO Box 337, Waxhaw, NC 28173, had its insurance license revoked. This action was taken based on allegations of repeated insurance violations.

Premium Choice Insurance Services (PCIS), 16501 Ventura Blvd., Ste. 400, Encino, CA 91436, had its insurance license revoked. This action was taken based on allegations that PCIS had violated Wis. Stat. 601.42 in two instances, had repeatedly violated insurance statutes and rules, and had demonstrated a lack of competence and trustworthiness. PCIS had also failed to pay or otherwise respond to a $1500 forfeiture assessed previously.

Rateforce LLC, 3423 Piedmont Rd., Atlanta, GA 30305, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Reliant Insurance Agency, Inc., 1600 Golf Rd., Ste. 1200, Rolling Meadows, IL 60008, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Sbxprss Inc., 5793 Widewaters Pkwy., Ste. 230, Syracuse, NY 13214, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Sunshine Direct Health Group Inc., 6635 W. Commercial Blvd., Ste. 207, Tamarac, FL 33319, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
